In our work, we often deal with patients suffering from coral kidney stones. This is the most severe form of urolithiasis, occurring in 10% of patients. Coral stones are large kidney stones that occupy a significant part of the pelvis and spread into the tubercles. To a large extent, they disrupt the work of the kidneys, are a source of the inflammatory process, pain syndrome. In most cases, this requires surgery. The danger of coral stones also lies in the latent course in the early stages: the absence of complaints does not give the patient a reason to seek help.
CAUSES FOR THE FORMATION OF CORAL-LIKE STONES Prolonged disturbance of the outflow of urine from the renal pyelocaliceal system (strictures of the ureter, pyeloureteral segment) in most cases leads to the formation of large stones. Other causes of stone formation are diseases of the endocrine system, such as hyperparathyroidism. Excessive excretion of calcium in the urine in such patients contributes to the rapid formation of stones from calcium salts. Metabolic disorders, in particular purines, are typical for patients with uric acid stones. Such patients are often overweight, diabetes mellitus, gout. Kidney stones of an infectious nature (brushit, ammonium urate) are formed as a result of prolonged infection of the urinary tract with bacteria that change the acidity of urine to the alkaline side. Such stones are prone to rapid recurrence after their removal. Coral stones are also formed in people with congenital kidney diseases and abnormalities of their structure, with tubulopathies.
The main treatment for patients with coral kidney stones is surgery. Percutaneous nephrolithotripsy is currently the "gold standard".
Under general anesthesia, instruments are inserted into the kidney through small skin punctures in the lumbar region. With the help of a laser, ultrasonic or pneumatic lithotripter, the stone is crushed and fragments are removed. The number of punctures in each case is determined individually, depending on the characteristics of the patient's anatomy and the location of the stone. The task of the surgeon is the maximum removal of all stone fragments with minimal damage to the kidney parenchyma. During the operation, visual, X-ray and ultrasound control of the removal of calculi is performed. The successful performance of such operations requires a highly qualified surgeon. The results of treatment are directly influenced by the technical equipment of the operating room: the use of a combination of X-ray and ultrasound guidance, miniature instruments, flexible endoscopes and the optimal type of energy for crushing the stone.
