Conservative myomectomy

Conservative myomectomy is a radical method of treating uterine fibroids by exfoliating the neoplasm with subsequent suturing of the tissues. Refers to organ-preserving operations, that is, does not affect healthy tissues and organs. It implies only the removal of the myomatous node. The uterus, its reproductive function, menstruation are preserved. This surgery is performed in combination with drug therapy or uterine artery embolization. Such a set of measures allows you to safely cope with benign neoplasms in the uterus with a low probability of recurrence.

An operation is prescribed if you have the following symptoms:

profuse menstruation and atypical bleeding occur;
it is not possible to get pregnant during the year of active sex life;
frequent pregnancy failures are observed;
the size of the neoplasm corresponds to 12 weeks of pregnancy or more;
myoma compresses adjacent organs and disrupts their functions;
myomatous node has atypical localization (on the ligaments, cervix);
the neoplasm is growing rapidly, causing bleeding and anemia;
myoma is combined with endometriosis or ovarian tumor;
the tumor tissue is dead.

The main types of surgery:

Abdominal myomectomy. It is carried out through gluttony. From a technical point of view, it is the simplest method of conservative myomectomy. The doctor makes a suprapubic incision in the abdomen, finds the knot, exfoliates it, and sutures it. The incisions are sutured in layers.

Laparoscopic myomectomy. The doctor gains access to the uterus through punctures in the abdominal cavity no more than 1 cm in size. Thus, endoscopic instruments are inserted. The used laparoscope is equipped with a video camera and lighting. This allows you to quickly find the growth and prevent the formation of unsightly scars.

Hysteroscopic myomectomy. It is indicated in the presence of a single node on the anterior or posterior wall of the uterus with a size corresponding to 16 weeks of gestation or less. It is carried out through the vagina using a hysteroscope equipped with an optical and video system.
